How to apply for Ayushman Bharat (PM-JAY) scheme?

identify: First of all, Arogya Mitra will check with the help of your name, ration card number or mobile number whether you are eligible for the plan or not.

Search in database: Information of your name is discovered in the database of SECC (SOCIO-Economic Caste Census), RSBY or State Health Scheme.

Uploading document: If your name is found in the database, then you have to submit government identity like Aadhaar or PAN card and family documents.
